Our approach to health care at each stage of the life cycle - prenatal, pediatric, adolescent, adult and geriatric - stresses health promotion and disease prevention at every age. We routinely treat adult patients suffering from dental disease, asthma, diabetes, hypertension, arthritis, cardiovascular disease, depression, sexually transmitted diseases and more. In addition to providing quality health care, our providers emphasize preventive medicine and patient education.

Our commitment to overall community wellness is further evident in our preventive health initiatives. We have embraced the goals of Healthy People 2010 to improve the health, fitness and quality of life in the communities we serve. In the past year, Ohio North East (ONE) Health Systems has expanded preventive services and enhanced the health education department.

Services (Varies By Site)

Services are available to patients of the community. Ohio North East (ONE) Health Systems, Inc. accepts many types of insurance including Medicaid, Medicare and some private/commercial insurance plans. A sliding fee scale is available for the medically uninsured that qualify.

Methods of Payment

  1. Medicare, Medicaid or insurance cards need to be presented at every visit so that health care coverage may be verified prior to being seen by the provider.
  2. This is not a free clinic. A sliding fee scale has been established to assist the medically uninsured with health care costs. Please refer to SFS section for more information.
  3. Payment for all services is expected at time of visit. Financial arrangements may be arranged.

Ohio Medicaid Card

  • Medicaid/Healthy Start Healthy Families - covers all services provided at ONE Health.
  • All Medicaid HMO's for example Caresource and Unison.
  • Ohio Disability/General Assistance - Covers medical services. FOR DENTAL it only covers tooth extractions and x-rays of the tooth to be extracted. For all other dental services the patient is eligible for the sliding fee scale with proof of income.
  • QMB/SLMB - are not medical cards, but coverage for Medicare recipients.

Medicare

This insurance covers those qualified 65 or older or those on SSI for medical services and flu shots. MEDICARE doesn't cover dental services. However, Medicare patients may apply for the sliding fee scale for any primary dental work.

Sliding Fee Scale (SFS)

Uninsured patients may apply for SFS to receive discounted charges for services.  SFS payment is based on total household income and number of people residing in the household.  An application form must be completed and submitted for approval along with proof of household income (Please refer to application form for acceptable proofs of income).  Verification of SFS status is updated annually and as needed.  Patients who do not supply proof of income are not eligible for SFS and are responsible for 100% of the charges incurred at the time of visit.  There are no exceptions to this requirement.

If qualified, the individual will be required to pay a portion of the bill ranging from nominal fee to 100% of the charges at the time of visit as follows:  

  1. Nominal Fee= Pays only $10.00 per visit, per service.
  2. 25%= Pays 25% of total bill per visit, per service.
  3. 50%= Pays 50% of total bill per visit, per service.
  4. 75%= Pays 75% of total bill per visit, per service.
  5. 100%= Pays 100% of total bill per visit, per service.

Private/Commercial Insurance

It is the Patients responsibility to check for network coverage.  Patients are responsible for 100% of the co-pay and deductible.

  • If ONE Health is out of network the patient is responsible for 100% of all costs incurred.  Additionally,   prescriptions, referrals and hospitalizations may not be covered if services of a non-network provider are utilized.  The responsibility of payment for any non-covered services falls upon the patient.
  • If the insurance is major medical (hospitalization only) or does not cover a particular service (IE: well child or dental) the patient may apply for the sliding fee scale.  

How to Pay for Services

Cash and personal checks are accepted.  If a check is written it must include the imprinted name and address.  Postdated checks are not acceptable.  We accept credit and debit cards.
